Good questions. And quite separate from whether a client cares if their escort cums or not. 

I can see reasons for the revelation being disconcerting. Part of what some people are buying is the illusion that it's a relationship, with an accompanying illusion of exclusivity. Without the revelation it's easier for the client to maintain the illusions. Even when they know that neither is true. Knowing the reality and accepting the illusion requires holding contradictory ideas simultaneously. Being confronted with the contradiction is buzzkill? Perhaps. 

Building the ability to accept the reality of the experience in the moment, while also being able to ignore anything that's at odds with it, including being told explicitly that the escort is saving his cum for someone else, I'd suggest is a skill worth learning. Being able to simultaneously hold two (or more) contradictory ideas is sometimes necessary.
