If your guy is already way overcharging for massage (like, $200+/hr), it’s safe to assume these services are already baked into the cost and I wouldn’t worry about it.
$160/hr or less I would assume nothing more than massage, and enjoy the possibility of some extras if he does them. Happy endings are very common and normal, but not guaranteed. Do not expect full service, oral, or your masseur to cum for you. Some guys might do these things for their regular massage fee, but please respect masseurs with boundaries around what they will do for their standard massage fee. Boundaries vary. Clients are not entitled to anything they didn’t already discuss with their masseur.

Tipping is supposed to be a thank you for services above and beyond the.standard.
Unfortunately like restaurants, masseurs in Spas are underpaid so your "tip" is much of their paycheck.
A private masseur will charge far more and doesn't need to give a cut to anyone else. So for very standard service no tip is required. Anything above and beyond:  the provider should be compensated with 20% to 25% depending on how far beyond the service went. Or round up to escort rates ($200 -$300) if sexual serviced were added to the session.

No. First of all, the communication problems makes it difficult to find out if they'll do what you want, or if they understand what they're getting into.

It sounds like the OP scenario was for an escort. The following is in response to @KensingtonHomo’s comment regarding masseurs.
Regarding the BJ, it’s perfectly reasonable for anyone to charge you for sex work (and that is what it is). It doesn’t matter who is giving or receiving. It’s all the same.
Massage has an appropriate rate for it depending on your local market. If we had no boundaries on what we will do for a massage fee, every massage would essentially be underpriced sex work, or overpriced massage. We certainly have the latter available with the $200+/hr massage guys all over. The former you will find masseurs not charging extra but being picky about what they do with whom.
If you don’t want them to be picky, pay for the services you want. If what he quotes you is too much, pass. Don’t expect a masseur to give it all away for free to anyone who asks…for a reasonable massage fee. When he does, be a gracious client and tip him generously.
Giving the option for extras, in my book, is fine. Nobody is begging you to suck their dick as if you’re doing them a big favor. The service is about YOU and making YOU happy, whatever it looks like for YOU.
Big caveat here. Upselling is unprofessional. If you (client) are asking for extras and the masseur quotes you, no harm done. He has every right to say no or offer additional services for a fee. Guys who are serious about the gym often like strict routines. It’s how they achieve amazing results. Some even see the gym as a job, it’s part of the work that enables them to be successful in everything else, including escorting.
