If he was hired with the understanding he'd be bottoming for you, then yes it's a deal breaker.  He should have been prepared if it was likely he'd be bottoming. At least he did give a warning instead of risking the mess.  If he was expecting to be the top (based on his advertised role or discussions with you prior to meeting), then it's more just buzz kill.

I'm not a doctor and don't play one on TV, but my personal experience is that if I eat anything substantial it will cause the rest of my digestive tract to become active.  I've always thought about it like turning on a conveyor belt, where everything gets moved down the line at the same time.

An this is also very true, but now that we know what was discussed before the appointment, we know that the provider was at fault. I'm inclined to think he had another appointment before and used this as an excuse to not perform that part of the service.

It's a combination of both. The gastrocolic reflex is naturally triggered when we eat, due to gastric distention. Think of "making space" for the incoming food. 

However, if the person is sensitive to a particular "food trigger", this can cause an acceleration of peristalsis, in an attempt for the body to get rid of the insulting foodstuff. This is called "postprandial diarrhea" and is a typical manifestation of some kinds of irritable bowel syndrome, for example (although this is not enough to reach that diagnosis). 

So, in conclusion, yes - while it is natural to do #2 after a meal, it can get messier if you ate something you are sensitive to.
