This wedding and similar events of excess covered as news leaves some people asking for another piece of cake while others starve. Speaking out against it does not seem outrageous to me.   4. How many patients did I hear say: "I would do anything to lose weight." It became clear that they would do anything, except diet and exercise. Fen Phen, remember that? Gastric bypass. Gastric sleeve. Tapeworm therapy. Every conceivable medication out there and now Ozempic and other medications which require ongoing injections and ultimately end in a plateau in the weight loss for many, without ever achieving goal weight. As a fat man, I have dieted with great success but life and circumstance have led back to regaining the weight. I am trying to lose again, the old fashioned way, diet and exercise. But after initial success with daily walks and a more controlled diet, an injury has slowed by progress. It should be noted, that my diet problems revolve around eating at the wrong time (before bed) and not eating properly during the day (skipping meals, not having breakfast for example). My basal metabolic rate has been slowed as a result of these bad habits which have been hard for me to change. Now that I am retired and live a more structured life, I am hoping once my stress fracture heals, I will be walking and exercising and eating properly again and that the long road back to a healthier weight will be easier to find. I do not see Ozempic or any other currently available medication in my future. But as weight loss medications are a gold mine for drug companies I expect that more and "better" medications will be around the corner.

Oprah has always sought attention and while she had an evert person personae, she has never shied away from the spot light. Well, except for all the secrets she keeps in one of her many many closets. In general, I think of her as a good celebrity but not necessarily a good person.
  6. Any evaluation of a patient requires as much information as possible regarding the illness and the patient. A physician should never assume that any one factor is the most likely cause of a particular medical issue, neither should they ignore pertinent data. If you do not supply the information or the physician does not request it, a disservice is being done to your health care. I believe it would be prudent to select a doctor based on one's experience with the doctor, rather than by considering with whom he has sex, any more than one would select a contractor based on the same date. If you wish to support gay professionals, make sure you select a well trained one with whom you have a good rapport. If you think your doctor is assuming that all your ailments are related to your sexual orientation or your smoking, or your weight issue, or your age, it is time to find another doctor, gay or not, who takes time to question you thoroughly about the illness and makes a well considered plan based on all the facts at hand.

  14. I see no reason to ask that question on a form and every reason to ask that question as part of the. history taking. I would usually ask the question in this manner. Are you sexually active. Most adults reply yes, many with the same old tired jokes. Not as much as I want to or some variation thereof. After the oh you're so witty moment has passed, the next question assuming a yes to the first is: Do have sex with men, women or both. Which is your preferred partner? This may lead to follow up questions such as when was the last time, how many partners etc. Other questions such as have you knowingly had unprotected sex with Iv drug users. Gay men. Those questions go to both sexes. If I sense any hesitance in the patient or I see that they are uncomfortable, I explain the need for an accurate sexual history related to possible exposure to sexually transmitted disease. Unfortunately, I have had more than several patients come in for a routine physical who say that they have never been asked those questions by other doctors. I explain that this is an essential part of the history but if they do not want to proceed, it would be best to find another practitioner though I will perform the rest of the exam at this time and note the request to not ask sexually related questions. I can think of at least 5 occasions in which the answers to the questions led to HIV testing and a positive test. I did not routinely order the test, but I always asked if they wanted to have the test done. Of those five occasion one was a pregnant woman who would have had the test done as part of the prenatal evaluation in any case, but she was very surprised at the result. Another was a man in his 70s who said he had not had sex in 10 years and he had not had sex with a man for slightly longer than that. His chief complain was fatigue. So while I think the question is clumbsily phrased and the whole situation could be handled more personally, I am pleased that at least the questions are being asked.

  18. Some people just look like a cop. My brother in his younger days was always being mistaken for a cop. He was tall, broad shouldered and carried himself with a certain swag that sang out "cop".   19. There is a closed thread on this series from Netflix. I watched it all yesterday. I enjoyed it very much. The characters were recognizable in that they had the kind of concerns many people face when dealing with interpersonal relationships. It was clear what the ending would be in view of Bruno's passion for rom coms. There is some nice eye candy and a few r rated scenes of gay sex. It seems as though they did not make a second seasons which I think is too bad. At one point in the show, one of the characters mentions that there is the excitement of getting a gift and then the how to deal with the gift once you get it. That is exactly what the second season would be about if they ever make it.
