In a choice-theory economics framework…. actually it’s not a terrible idea to systematically charge clients higher for service that is not just riskier to him, healthwise, but could put him out of work temporarily.

There's no such a thing a "incurable syphilis". 

Increasing (to the point of almost universal) macrolide resistance? Sure. But that's a long way from "incurable".

Good old penicillin works, and for some reason, it (thankfully) doesn't seem to be able to develop resistance to penicillin.
