I have made small loans to friends and family.  I always insisted that they sign a note - and I always got repaid.   Once, my mother asked for a largish loan - to pay tuition for my brother and sister in private schools.  Since their state had perfectly good public schools - I told her "no," hard thing to do, but I knew I would never get repaid.

I have relied on the kindness of client-friends many times.  Without my closest client//friends I'd probably be homeless, or have had to resort to a real job (shudder!).  And yes, it could make me feel like a worthless whore to have to ask for help.  At the very least I feel a BIT disappointed in myself.  But Jesus Christ no matter how awkward I feel I continue to thank them, years later.  I'd never ghost someone who helped me just because I feel embarrassed.  Who would.   Someone who isn't doing well. Sucks dude, nice of you to help him.  VERY nice of you to still be concerned.

Perhaps another view here is that you may still trust him and have reaffirmed in your communications that you are not looking for reimbursement; however, I wonder if the trust is equally reciprocated, especially if the initial discussion about the transaction was based on "repayment" - even though there was no set due date for it. I also wonder if the transaction was contextualized instead as a "gift," whether he would have been more likely to respond to you.

Do you know of any explanation as to why the visit wasn't covered? As ER visits go, though, a bill of $4200 isn't that bad; my last one was $14K+, although after Medicare and Supplemental adjustments, my share was less than $500.
I know insurance companies do not like their insured customers going to an ER for non-life threatening or otherwise less serious conditions, and many people use them in place of a regular primary physician. ERs are notoriously -and ridiculously- expensive and visits to a doctor's office are almost always much cheaper. For this reason, his policy might have severe limitations for ER coverage, including a significantly higher deductible.
Of course, the escort was in a tough situation, being away from home base, with no local connections for medical care/doctors, and in pain. And since he was away from home, he would almost certainly have been out-of-network, if that applied to his coverage.
He might talk to his insurance provider to better understand their reasons, and if they don't appear to be reasonable or contractually compliant, he might contact his state's insurance regulatory office for advice or to file a complaint.

I would say no.  There are plenty of guys who are looking for shorter guys.  Work on your body in other ways, it will ache from doing those exercises.  However those will pay off.

This surgery while essentially cosmetic has economic gains attached to it.  Taller people are viewed as more deserving of promotions especially in some Asian cultures.  It seems ridiculous to undergo painful surgery to add a few inches to one's height, but there are many people who will try to advance their career in any way they can.

I've known someone who went through this.  The scarring on his legs was awful.  He said the pain was immense and that his surgeon said a lot of the people who go through this end up with pain pill addictions.  
Height is one of those born-with things I'm very, very thankful for.  It's served me well.
I'd agree in this instance that just because you can doesn't mean you should.
