Never... the risks is there but you're highly unlikely to get a disease that way. Unfortunately now because of PrEP a lot of guys are fucking without condoms and getting diseases (while so far avoiding HIV) and that might increase the risk of getting something by oral sex because there are more guys who have STDs.

While you are partly correct that most head/neck cancers are caused by tobacco and alcohol use, there is a distinct subset of oropharyngeal squamous cell carcinoma which is associated with HPV infection. This particular subtype has seen a recent increase in incidence over the last decade or so, and it is felt to be different from the run-of-the-mill cancers caused by tobacco/EtOH in terms of its biological behavior. Fortunately, it has a better prognosis than the usual type that occurs in alcoholic smokers.

In my opinion, not only as an expert but also as a gay man who prefers *not* to use condoms for oral sex, everybody should get the HPV vaccine. Depends how you're doing it. Trolling the bathouses/revolving door hook up site sluttiness then yes. Doing it here and there in normal settings your risk is very low but of coarse not 0% and like you say about picking up stuff easily treatable. That's my experience between those two environments. I went from picking stuff up way too often to a 0% for many years now.

 

The only time I would wrap it up for oral is if I had a lover at home I needed to protect. Other then that it's completely ridiculous. Might as well become the next bubble boy and lower your life risk to % then.
