Not necessarily. The provider sets the rate. If they're willing to go beyond typical sensual massage at that price, it's their choice. I do and would encourage a nice tip. 
Perhaps some providers can shed light here, but I have been surprised pretty regularly by a massage where the last 20 minutes turned into full-on sex. I didn't request it, but I consented. 
In my experience (I'm a nice-looking, middle-aged guy in good shape), some providers are just into it at the moment.

I agree with the above suggestions, should you find yourself in this position again:  Arrange for him to leave as soon as possible, tell him why you are dismissing him early, and then leave a negative review.  If he was just being entitled but not threatening to withhold service that's the basis of your engagement, I would suggest just having a frank discussion.  With someone like this, though, that's just throwing good after bad since I doubt he would perform well once his demands weren't being met.
I'm puzzled that anyone would hire someone he's never met before for a longer than overnight session (and even that is a risk for an unknown escort).  Or, at the very least, it seems like a more detailed discussion regarding expectations needs to happen before the trip is even scheduled.

Agree and he should not advertise for sensual massage and include shirtless pics in his ad if he is going to only offer therapeutic massage while fully clothed. Lots of other choices at that price point that deliver exceptional therapeutic/erotic sessions.

That rate seems high for purely therapeutic session.
