As someone who works in healthcare, our Healthcare system is designed by rich white STRAIGHT men, for rich white STRAIGHT men.  We put the best hospitals ckinics and clinicians in affluent areas (we call it a "good payer mix", payers being insurance).  We schedule services during normal working hours.  Majority of information is in English, and staff are English speaking.  We don't really attempt to understand peoples' cultures, norms, preferences, or unique factors that affect their health.  Research dollars disproportionately go to solve problems of rich white STRAIGHT men.  Visiting hours don't accommodate ppl who work odd hours, or multiple jobs.  It's hard to even get a routine lab draw done outside of "normal working hours".  I could go on and on.

    I'll offer you an example.  I had a minor surgery recently where I stayed overnight in the hospital.  I brought my usual daily meds with me including my PREP (Truvada).  Well, the fact that I brought my own meds triggered a huge drama.  A nurse manager was called, then a pharmacist was called to the room.  They held the plastic ziplock bag up "what's this blue pill"?.  They took the pills away for investigation.  They came back 2 or 3 times to ask more questions.  Meanwhile, they must have said the word Truvada (at full volume) what felt like 100 times....in my 10X10 room with a STRAIGHT roommate whose family was visiting.  It sounded and felt like Truvada Truvada Truvada Truvada Truvada Truvada Truvada Truvada Truvada Truvada Truvada Truvada Truvada.   All while recovering from anesthesia.  No consideration about my privacy, or that being on that particular med might be sensitive information.  THAT is what a healthcare system designed by rich white STRAIGHT men looks and acts like.

Welcome to the Company of Men, and your new adventure!  Wanted to reinforce the notion of unprotected sex and the education and precautions you will need before going down this road.  If you do not have a gay or gay friendly doctor, see one even if it's not your PCP.  Many major cities have a health clinic that specializes in the unique issues and needs of gay sex.   These clinics are just like any other doctor's office, there is no stigma to going there.  You may also seek these services via Telemedicine if you are not comfortable going in person or these services are not available where you live.  There are a handful of vaccines you will want in addition to PREP, also be aware there are one or more STIs for which there is no prevention or cure (Herpes for one).  

    I'm not intending to dissuade you from the path you are on, you are meant to be here.  Just encouraging you to go down this path fully educated and protected.

The way you describe your siblings, they seem to have some level of mental illness (addictions, etc.).  Does not change your course of action, but this is more complex than someone just making bad choices and acting like a teenager.  I lost my brother 2 years ago to a lifelong battle with alcoholism, and I had similar struggles over the years with him (not about money, there was none....just about his behavior and how that impacted his life and the family).  Still, with addiction there is literally nothing you can do until the person is willing and able to help themselves.  

    My father too had some sort of difficulty with money.  He had a successful career and a generous retirement, but he never had any money - ran up credit cards, payday loans, and asked me for money a few times.  I think he had a gambling addiction, the closest I ever got to an admission was that he played the lottery.  The last time he asked me for money I said no but offered to help in other ways (like set up a budget, hire a financial advisor, etc).   I did this after drawing up a "contract with myself" of what my baseline principles were for helping, and what I was and was not willing to do.   For example, I would not allow my parents to be homeless or hungry.   I also would not put my own money/retirement at risk.  I would not enable my father's destructive behavior.  I did buy my dad a used car (10k) and put it in my name so he could not borrow against it to ensure he had transportation.   I also helped twice with refinancing of their mortgage to provide some relief with monthly expenses (just helped with the paperwork, dad was in his 80's by then).

    I hope the mental illness angle allows you to come at the situation with some compassion, while not sacrificing your principles.  Consider writing up that "contract with yourself" - it will provide you clarity as you face decisions over time.  If you have some flexibility with how the trust is administered, you may be able to set up structures which pay for basic needs and nothing more - ensuring he will never be homeless or hungry, while protecting your assets.

Given uncertainty of weather, I wonder if it makes sense to wait until @ 1 week before and then make your travel plans?   I also live in the NE - and weather forecasts here are not that reliable at t+7 days, but in other areas of the country should be.  Looks like it will travel directly over Niagara Falls.....

    Thanks for this validation.....I have been reading up on Intermittent Fasting, and am planning to start....though plan a "skip breakfast" regimen (timing just works best for me).  Skip breakfast, high protein/low carb lunch and regular (reasonable) dinner.  Glad it's working out for you.....Jack

  10. I think the statistics still prove that your earning potential is much higher with college (or trade school) education. It is true that things have gotten much more competitive at the Ivy level, however, there are still great affordable choices in the state schools. I am a VP at a large organization, started my career and community college, went to work, earned my bachelor's at night (having my employer pay for half of it), and just a few years ago completed my MBA and a top state school program, which cost $39,000 and I paid cash.  Point is, there are lots of pathways to education…. We tend to think of higher education as a binary thing… Go directly from high school, go to top school, have mom and dad pay. There are many alternative, affordable, and accessible choices out there.
