you can never fully trust anything about someones status - that is why you must be responsible for your own health and be taking prep or using condoms.
I have had scares when fucking women who assured me they were on the pill, and I trusted that. With prep, I like that I do not have to worry about whether my partner is taking it or being honest about their status - all that matters is that I am taking my prep every day.
I can understand why some providers are not honest about their status due to miseducation and the massive fear campaign around HIV that we (rightly) grew up with. Be responsible and use protection when you are with anyone - whether that be condoms or prep.

Even if they’ve always said they’re negative, I’d still take it with a grain of salt and look out for yourself.
I’ve seen people go from positive to undetectable, then no mention of anything, then back to undetectable, then on PrEP. It can be all over the place.
At the end of the day, I’m on PrEP, doxy PEP, and I use condoms sometimes. I just make sure I’m in charge of my own protection. And I’m the one who decides the level of risky I’m taking 🙂

There are rare cases in which an HIV positive patient seroconverts to HIV negative. These are usually related to stem cell transplants for other conditions.

When I started hiring I was in my 30s and reasonably attractive (I got plenty of sex for free), so I didn't think about whether the provider would be turned on or off by my appearance. However, by the time I reached my 60s, I began to wonder if the pleasure really was mutual or the provider was only going though with the act for the money, and it bothered me enough to eventually stop hiring. It sounds like you are really self-conscious (and uncomfortable) about your own appearance, so as others here have said, it is important that you find a provider that is not going to react in a way that will ruin the experience for you by confirming your fear of rejection. And as several others here have said, the best way to insure that doesn't happen is to be completely upfront about whatever it is in your appearance you think someone would likely be turned off by, so the provider is prepared for it: weight, scars, other kinds of disfigurements, even handicaps. And as has been said here often, cleanliness is the most important issue for many providers. (BTW: don't forget to brush your teeth and use mouthwash.) What you don't want to do is apologize for something after you meet him. If you have prepared him, and he has accepted the appointment, then relax and enjoy yourself.
Note: I don't think that sending him a photo is a good idea. He is the one auditioning for a role, not you.

When I get a massage I tip the same whether it's a spa or the guy's own place. Somewhere around 20%. The spa I go to provides an envelope with the masseur's name on it, and you drop it into a box on the way out, so hopefully that means they're not skimming off the tips. But I can't really control what happens to the tips in a spa, so just hope the masseur gets compensated fairly.
To @jeezifonly's points, the spa covers the overhead, but the pay tends to be way less than what the spa charges. This is true of trainers at gyms too... my trainer at Equinox let me know what their compensation structure was, and it was surprising how small a percentage they get from what the gym charges, unless they have lots of clients. But having overhead covered and a good place to conduct business is significant too.
So if you think about it, the masseur in the spa has his overhead covered, but is being paid less. The guy with his own place is keeping his entire fee, but needs to cover his overhead including providing a decent place for business. Maybe that's a spare bedroom, and that has a cost associated with it. So I'd say the same amount for tips regardless.

At a spa, the bodyworkers are usually hourly/daily wage-eating employees, with taxes withheld. Most legit ops require proof of education and certification in the field, which is at the prior expense of the worker. They are on call with clients assigned, all overhead and expendables covered, and tips for them can go a long way to get them through hours cut in slow periods. If they have repeat requests by name, more hours will come their way. Some may also do private legit bodywork on the side, but some may not have a home setup for it, and others may even be concurrently attending school for PT or other further medical training, without the time to earn more than the spa pays.
If you're not inclined to be a service tipper, then don't. It's not really a problem until your next visit...
