This sums it up for me. They've got to actively be touching my dick, hole, or rubbing their whole body on mine for me to start getting aroused usually. Interesting that the consensus is for 90 minutes.   Me too.   In my experience, the massage rarely begins on time, so the actual time on the table is typically less.   And I also prefer a little break in the middle for some water.

In Canada we have a system called Interac which I have used a couple of times. Once when I forgot cash for a massage and had a provider who preferred it. Otherwise it's always cash. Most ABMs in Toronto disburse 50s and a number of then do 100s.

The key is that full sex should not be expected by the hirer when hiring a sensual or erotic masseur. As for what’s included in the massage as ‘standard’ you should ask the masseur. I include things like oral and kissing as standard but many don’t so it’s incumbent on the hirer to ask. 
For many clients though the interest in hiring a sensual or erotic masseur (apart from receiving a great massage) is the possibility and unknown about what might happen. That’s exciting and adds a frisson to the session. I do have sex with some clients because that’s just how the session developed. But as soon as a client wants the guarantee of full sex when they book the session then I am going to tell them that the rate is different (if I’m willing to do it atall). Clients expecting/demanding full sex for the price of a massage are unrealistic and will be disappointed. 
 

This is key. Whoever is offering services has the right to establish their boundaries up to and including during the session. Clients need to take providers at their word and NOT try to pressure them into other things. In this particular case, @ProudWhore should block this guy because he's not respecting the clear boundaries. 

Well, in my experience, there are times when fantasy does match reality. I have experienced an entire range of encounters with different masseurs, ranging from a simple (and great, or terrible) massage, to a massage that feels sensual but not sexual, to happy endings (meaning getting a handy jay), to oral, to anal, to almost braking the massage table having sex in all kinds of sexual positions.
 In my experience, and let me say it again, IN MY EXPERIENCE, the difference is not about sex happening during or after a massage session; it's about the masseur being the one who calls the shots. He decides how far he's willing to go. Also there are certain types of settings that many masseurs have:

Different locations/countries use massage services as a cover for escort services, including the USA. There's no rule, and as such, I find it ok to sometimes ask (me as a client) about what additional services, if any, are offered by the masseur (you'll be surprised what they offer via text). So I understand that you are being clear about what you offer, but I also think that it should not be denied, that many other providers do go farther, do offer more for more, or just like having sex with the clients that they like.
As a masseur, you do call the shots, you state and decide how far you will go, and your client should respect your boundaries, but there's no rule about it, in my experience.

I pay after and place the folded-up money in the masseur's hands--often with a tip added to the agreed-upon amount--look him in the eyes and say "Thank you." Once, a very Zen-like practitioner squeezed my hands when I gave him the money and said, "May this come back to each of us a million times over." I thought that was lovely. I tried to say it myself a few times, but all I got out was "I wish this was a million dollars because that's how much you're worth." Not as spiritual, but I think it was appreciated.
Oh--and I always turn away to finish dressing after I give him the money. If he looks or counts it,  I don't want to know. More often than not, he just puts in in his pocket or the dresser.
