Unless specifically discussed/requested I don't expect the provider to cum but when he does (or they do) I consider it an added bonus. A few times I've been fortunate enough that the provider has cum more than once.
I find the odds of a provider cumming are greater (based on my own personal experience) if the appointment is late morning or early afternoon before he has had too many other appointments. Definitely not a guarantee but it has worked in my favor.
