Getting enough sleep is paramount, and with my work schedule it's a big reason I struggle to loose.  
Also- there are a myriad of reasons people may be overfat, including genetic conditions.  

Especially because most providers don't ask for pics beforehand, they literally don't know what to expect.  Also depending on one's exact build even 20% bodyfat can look VERY different on two people who have different muscle mass.
