But ....
He's a bit of a "broken toy" emotionally. He shared some broken heart stories..and seems to have some serious issues about sexuality. I had the same experience as others where the gatekeeping about sexual lines in the sand felt like a minefield that I was sorry I had stepped into.
I don't expect a provider to go further than a hand job for an erotic massage....and since he seemed like a sweet guy with a little baggage..I figured I'd tip him well and see where things go on a second meet.
Normally when I book a second massage with a provider there's a more relaxed environment and an eagerness to please.  In this case it was the opposite. He was even more remote, robotic and just plain weird. Made me feel like a dirty old man for asking him about his thoughts on escorting.

You're correct. I don't know that they're taking care of their health, and I'm protecting myself in case they don't. I think it may be more likely that they're doing that than someone I might meet at a bar, because it's in their professional best interest to do so, but I'm not counting on it.

Totally agree. I have preferences around shape, girth, direction. An as a bottom, I'm not interested in pushing my limits. 
I find that 7-9 is my sweet spot, depending on other factors.

I don’t mind handing the envelope with the cash inside directly to the provider. It’s always as we are about to part or leave the hotel room if we are going to breakfast. They invariably graciously accept it and thank me.

Once it’s in then I ask how many thrusts the client wants. They can buy the Platinum service (£100 for unlimited thrusts) or package of 20 thrusts for £5. The final pull out is free. If they want to change position then that’s like buying a new ticket: they need to pay by the inch again. 
If I’m bottoming I don’t charge by the inch (it wouldn’t maximise income from less well endowed clients…and if I find someone with 12 inches then he’s welcome to use them all for a flat rate tbh).
