I faced a similar situation, returning to an active sex life after a long dormant period. I talked to my doctor, and he started me on PrEP and Doxy PEP, and I get tested every three months. I have sex with providers only, and one of the reasons is that I know that they have to take care of their health. My regular provider and I have actually never felt the need to talk about this. I know that he's responsible for his health, and he knows that I'm responsible for mine. We have really great BB sex. Of course, if I wanted to use a condom, or if I wanted him to use a condom, that wouldn't be a problem.
Also,

He's very sweet, very handsome and does a good massage.
But ....
He's a bit of a "broken toy" emotionally. He shared some broken heart stories..and seems to have some serious issues about sexuality. I had the same experience as others where the gatekeeping about sexual lines in the sand felt like a minefield that I was sorry I had stepped into.
I don't expect a provider to go further than a hand job for an erotic massage....and since he seemed like a sweet guy with a little baggage..I figured I'd tip him well and see where things go on a second meet.
Normally when I book a second massage with a provider there's a more relaxed environment and an eagerness to please.  In this case it was the opposite. He was even more remote, robotic and just plain weird. Made me feel like a dirty old man for asking him about his thoughts on escorting.

You're correct. I don't know that they're taking care of their health, and I'm protecting myself in case they don't. I think it may be more likely that they're doing that than someone I might meet at a bar, because it's in their professional best interest to do so, but I'm not counting on it.

Totally agree. I have preferences around shape, girth, direction. An as a bottom, I'm not interested in pushing my limits. 
I find that 7-9 is my sweet spot, depending on other factors. 

As a client, I always require a deposit 😜
