May sound like a dumb newbie question, But what does “clothing optional” entail when staying in a private home?

Yes.  Or, the host may keep his clothes on but allow the guest to be natural, if he wishes.

I've been eyeing Mr B&B for travel lodging.  Some of the ads explicitly say that clothing optional only means nudism is tolerated.  A few of the ads came close to hinting that the hosts might be looking for sex partner guests.  

A few ads even include a section labeled: LGBTQ+ Kinky Amenities like this one in Austin, TX.  But you have to pay a $79-per-year subscription to have access to that area. (I haven't.)

That's more likely to happen if it's both clothing optional and a shared bed with the host.
