Leaving for Hawaii in 2 weeks. The state is fine tuning it's testing program. It's not perfect, but it's a better screen than not doing anything at all. They are very aggressive in enforcing quarantine and arresting those who break it. ? Visitors are the biggest risk of brining the infection to the islands. I think it's great they are encouraging visitors to get free 2nd test post-arrival to make sure you stay safe. Visiting two islands and will be tested each time before arriving on each island. I hope they can improve testing to the point they can get results fast enough to prevent people from getting on planes that are infected to protect everyone traveling.

The rapid DFA tests are less than 70% sensitive, particularly at finding early infection. Better if done before leaving, and then again after some 3 days of quarantine on arrival. The PCR tests are much more sensitive, over 90%, but cannot be done so quickly. Another strategy which could work would be to require a DFA right before leaving, then a PCR on arrival, staying quarantined until the PCR results are available. I'm just about to fly back from my Puerto Vallarta trip. The Jalisco government took some steps I found somewhat counter-productive. Last dinner service was 7:30 PM, and the restaurants had to be emptied out by 9 PM. It just forced everyone to eat around the same time, and seemed to just concentrate diners by forcing everyone to eat at the same time. Then they closed the beaches at 2:30 PM. I didn't follow the logic there. It just forced everyone to the pool, again concentrating them instead of spreading them out. Then they put space blockers into the pool. While it did reduced the total number of people in the pool, it forced those in the pool to squeeze closer together. It would have made more sense to me to require fans around the pool, to force air movement in case the wind died down.

The Colombian man I was with had a really early AM flight, and I have a late PM flight. When I got back from breakfast, some young Cuban dude we'd met at the pool knocked and the door, and when I opened, he grabbed my hand and made me feel his massive cock. I quickly declined and closed the door on him. If he'd only known I prefer smaller wieners. I did almost pull a "Lindoro" on one of the hotel employees though. I thought of giving him my name, e-mail, and What's App number. I was pretty close to doing it, but decided against it. I'm kicking myself a bit now. Lindoro and I do have another dinner date in about 10 days. He keeps acting flirty but cool. Maybe the 3rd time around he'll be my Shipoopi?

The testing strategies you suggest and other measures seem sensible but one wonders whether anyone is receptive at this point. As I see new cases skyrocket in the USA and other countries, people are obviously not doing as much as we seemed to do 6 months ago which flattened the infamous curve the first time around In many countries. Not in the US though, which has seen more cases now than any other countries, even large ones like China and India.

 

As we enter the winter months, I fear the Covid virus at this point more than the seasonal flu, which I have always got vaccinated for over the last 20 years. It also seems it will be less widespread this year for various reasons. At least we had a respite this summer in Canada from the Covid virus which I took advantage of to see some of my favourite escorts.

Some states a lot more than others. California has done well, since it has very much limited indoor activities and acted on a county-by-county basis. I saw a dramatic drop in the percentage of my patients with positive results. Crowded venues, especially indoors, seem to be a major problem. The Sturgis rally in South Dakota seems to have led to a huge jump in Covid cases and over-run hospitals in the Dakotas. We didn't go to any bars or indoor restaurants in Puerto Vallarta. Face masks, sanitizing, and temperature checks were required to go into any store (and they were capacity-limited). I felt pretty safe. Even outdoor restaurants had fans, which are probably very useful in dispersing droplets. Crowded bars, discos, and churches seem to be major problems. Last week Icelandic scientists published a major study showing that, at least in that country, the case-fatality rate is about 0.3%, which isn't that different from seasonal flu. Major difference, of course, is the availability of flu vaccines. Covid vaccines better come out soon. People are getting fed up waiting.

The news seems to be encouraging today on the prospects for an effective vaccine pretty soon. If this works out well, the next question will be how soon everyone will be able to get a shot. Here in Canada the federal government has a deal with Pfizer for I believe 60 million doses which would cover 2 shots for the entire population likely to want one. And they are pledging to make them free of charge. From a public health point of view, this is the way to go. Just as our Covid tests are free to everyone.

 

not sure what the US intends to do but that will be up to the BIden Admin to determine.
