Interesting article in the NY Times: Nearly all Americans hospitalized for monkeypox infection had weakened immune systems, most often because of H.I.V. infection, the Centers for Disease Control and Prevention reported on Wednesday. Of 57 hospitalized patients described in the report, 82 percent had H.I.V. More than two-thirds of the patients were Black and nearly one-quarter were homeless, reflecting racial and economic inequities seen in the outbreak overall. The finding suggests that although most cases of monkeypox are mild, doctors should test patients with suspected cases for H.I.V. as well, and be prepared to offer prompt treatment for both infections. “Monkeypox and H.I.V. have collided with tragic effects,” Dr. Jonathan Mermin, the C.D.C.’s lead scientist on monkeypox, said in a statement. Most of the patients in the study were given tecovirimat, or Tpoxx, but treatment for some patients was delayed by as long as four weeks after they first sought care. As of Tuesday, more than 28,000 cases of monkeypox had been reported in the United States, and nearly 76,000 cases worldwide. A vast majority are still among men who have sex with men, according to the C.D.C. The number of new monkeypox infections has declined steadily since September. But the number of high-risk people opting for vaccination has also dropped. Only 7 percent of the vaccine doses administered so far have gone to recipients who are Black. People living with H.I.V. or with other conditions that weaken the immune system fared poorly in previous outbreaks of monkeypox in African countries. In the new report, C.D.C. scientists analyzed case reports from 57 patients older than 18 who were hospitalized for monkeypox between Aug. 10 and Oct. 10. Roughly 95 percent of the patients were male. All of them had skin rashes, and most also had severe lesions in the mouth, urethra, rectum or vagina. About one in five experienced symptoms in their lungs and eyes, and in four patients the brain and spinal cord were affected. Four of the 47 patients with H.I.V. were taking drugs to suppress the virus before they were diagnosed with monkeypox. About one in three had a CD4 count — a proxy for the immune system’s strength — of less than 50, indicating severe immunosuppression. Two of the patients, one of whom had H.I.V., were being treated for cancer; three were solid organ transplant recipients; and three were pregnant. All of these conditions are linked to a weakened immune system. One third of the patients were admitted to intensive care units. The Washington Post today debunks the story: But Ladapo’s recommendation — extrapolated from a short state analysis that has not been peer-reviewed, carries no authors and warns that its findings are “preliminary” and “should be interpreted with caution” — was swiftly condemned by medical and public health leaders, who said the Florida surgeon general’s announcement was politics masquerading as science and could lead Americans to forgo lifesaving interventions. More than a dozen experts interviewed by The Washington Post — including specialists in vaccines, patient safety and study design — listed concerns with Florida’s analysis, saying it relies on information gleaned from frequently inaccurate death certificates rather than medical records, skews the results by trying to exclude anyone with covid-19 or a covid-related death, and draws conclusions from a total of 20 cardiac-related deaths in men 18 to 39 that occurred within four weeks of vaccination. Experts noted the deaths might have been caused by other factors, including underlying illnesses or undetected covid. “We’re talking about a very small number of deaths. An extra death or two would potentially change these results,” said Robert Wachter, chair of the department of medicine at the University of California at San Francisco and co-author of a patient-safety textbook used in many medical schools. “I’m hesitant to even call it a paper; it isn’t published anywhere. The study above itself lists many limitations on its credibility: Discussion/Conclusion In this statewide study of vaccinated Florida residents aged 18 years or older, COVID-vaccination was not associated with an elevated risk for all-cause mortality. COVID-19 vaccination was associated with a modestly increased risk for cardiac-related mortality 28 days following vaccination. Results from the stratified analysis for cardiac-related death following vaccination suggests mRNA vaccination may be driving the increased risk in males, especially among males aged 18 - 39. Risk for both all-cause and cardiac-related deaths was substantially higher 28 days following COVID-19 infection. The risk associated with mRNA vaccination should be weighed against the risk associated with COVID-19 infection. Limitations These data are preliminary, based on surveillance data, and should be interpreted with caution. The results have several limitations: While this method has been used to assess risk of death following COVID-19 vaccination,2 it violates the assumption that an event does not affect subsequent exposure (for mRNA vaccines), which may introduce bias.6 Further, it does not consider the multidose vaccination schedule required for mRNA vaccination. This study cannot determine the causative nature of a participant’s death. We used death certificate data and not medical records. COVID testing status was unknown for those who did not die of/with COVID. Cardiac-related deaths were ascertained if an ACME code of I3-I52 were on their death certificate, thus, the underlying cause of death may not be cardiac-related. The finding that the Janssen vaccine was more protective than mRNA vaccine against mortality within 28 days of vaccination could be due to confounding and needs to be further evaluated. It is likely that the populations who received COVID-19 mRNA vaccine and the Johnson vaccine are different, something we were not able to ascertain in this analysis. It is possible that the population who received the Johnson vaccine was younger and healthier than those receiving the mRNA vaccines. The Pfizer and Moderna mRNA vaccines were released more than 2 months earlier than the Janssen vaccine when the recommendations were limited to those 65 and older. Additional studies should be conducted to further understand the risks and benefits of vaccination of males between 25-39. Increased risk in the primary analysis for the 25 - 39 age group was based on a small sample size. Additionally, significant mortality from diagnosed COVID-19 infection occurred among all adult age groups. COVID-19 mortality among asymptomatic or undiagnosed COVID-19 infection is less clear. However, excess overall mortality among 25–44-year-old Americans was significant in a study1 looking at mortality from January 2020-October 2020. The largest increases were among Hispanic and Latino. It is unclear what the contribution of asymptomatic or undiagnosed COVID-19 infection is to mortality risk, and how this contributes to excess mortality. 1 https://www.cdc.gov/mmwr/volumes/69/wr/mm6942e2.htm Confounding by age may be present in the 60 years or older age group, which may explain the slight elevated risk for cardiac-related deaths following vaccination. This may also explain the increased risk for the entire vaccination analysis group for cardiac-related deaths since this group comprises the vast majority of deaths. Removing those aged 60 years or older yielded non-significant results for cardiac- related deaths following vaccination (RI = 1.15, 95% CI = 0.99 - 1.34), mRNA vaccination (RI = 1.17, 95% CI = 1.00 - 1.37), and males with mRNA vaccination (RI = 1.09, 95% CI = 0.89 - 1.34). Lastly, this analysis was conducted during the first months the vaccines were available. Both COVID-19 mortality due to infection or risk of mortality associated with vaccination have likely changed over time. In the fall of 2022, most people have either been vaccinated or have natural immunity to COVID-19. Many have had multiple vaccine doses, multiple infections or both. Research to assess the current risks and benefits of the COVID-19 vaccine to help update vaccine recommendations should be studied in this context.
