It doesn't help that certain clients constantly insinuate that everyone who hires except them is ancient, obese and disfigured. 

Well.....considering that the police just busted a couple of guys in a hotel a few blocks from my home (one that is used by many of the escorts who come through town), I'm not sure it's accurate to say police don't care.  If I remember correctly, it's happened quite a few times in Texas recently as well.  I'll stick to the anonymous route.  

Those guys haven’t quite got the idea of what the job is. Definitely right to move on. 
Sometimes I get clients sending me a description of themselves and they ask if they’re suitable to book. They might even include a photo. They have this perception that they need to be ‘fit’ to come for a session. I tell them they only need to be healthy, as in no health conditions that might be a problem, but that their physical appearance is immaterial.
This belief that you need to be conventionally fit arises because so many bad sex workers have criteria or age caps on who they will see. I don’t think those guys are worth booking. Your session will be all about them. 
 

It is an interesting point.  I have visited several masseurs/escorts hosting at very high end rental apartments on West 42 and in the Hudson Yards area.  All of these buildings have doormen who call up the tenant and then unlock the elevator to get you in.  How many times per day or week do they call up and say, Eric your guest is here?  For the most part, gentlemen visiting for 60 or 90 minutes, on average.  Of course they know what is going on.  And so does everyone else who works in that building.
On the opposite side of this, there is a masseur I visit frequently at his West Harlem apartment.  One day, one of the tenants, an elderly lady, said - there is only one reason men like you visit this building and he lives on the 4th floor.

I have scoop.  He's currently visiting Kansas City.  He has been in the US from Russia for under two months.  Bed massage, which is never my favorite.  However, excellent technique for the massage.  Strong, with obvious training and experience.
Nice guy, but a big language barrier as English is new to him (even though massage isn't).  He is a little less defined than in his pictures, but still very fit and he has been blessed.  Nude throughout the massage.

I think the overall problem is lack of empathy. It’s not restricted to providers. 
And how many older gentlemen, when turning down those they don’t want to be with, do so with the respect they crave or relax their criteria to accept a proposition - as they wish / expect from the younger ones.
