Leave underwear on and cover it with the draping before he comes back in. That is normal. Don't say anything about pants off or lower back pain or whatever. If he asks why no pants, just say they're too constricting. Mention he did a great job yesterday and it felt great and you're back. Don't mention anything sexual. He'll lead if he wants to do something, but don't insult him by being too obvious. My earlier suggestions upthread are all you should do.

You can use terms like "donation" and "fee for your time only" with him in an attempt to make him comfortable with the $ question. Don't wait until the meeting to find out! Might want to say, after you find out the fee, that you don't want to talk money AT the meeting at all and confirm that you'll pay at the end of the meet.
