While I have not got to view them in person, Vin Marco has been bicycling up to 100 miles a day and leg pressing as much as 1100 pounds and his legs look amazing in his texts. The best laid plans....unfortunately my dog Bubba has been ill for awhile. He has a tumor called a chemocytoma which is a slow growing cancer near the heart. Its location causes him to go into heart failure and have his abdomen fill up with fluid. He needs to have the fluid drained every three weeks. I thought I could convince myself to leave him with a good friend or a relative, both of whom he knows. As time has grown closer, he has worsened and I am not sure he will make it through the week I was planning on being away. So unfortunately, I have cancelled my plans to attend. I hope you all have a great time. This is the second time I have had a last minute illness derail my plans to attend. The last time I was the one that was hospitalized. With the fluid removal, this will be the fourth time, Bubba usually bounces back, but like a rubber ball, each subsequent bounce is not as high and not as strong. I will be thinking of you. Oh I quoted him in the chart in case this situation does go further. I agree it was a total horror show. I have spoken to the Chief of Medicine and have asked that the ER portion of the care be reviewed. As for myself, by the time I collected the information I called for them to cancel the case but the patient was already in the OR. Ultimately I was supervising the residents and I could have stopped delayed the case until I had all the data so I am not blameless in this matter. I think there is a whole lot of blame to go around. To reiterate, I found these things disheartening. I did not take the first remarks as anything other than the very toxic words of a manipulator. But the kind of mind that comes yp with that is disheartening and the ease with which he said it made me think that he has said these words of some version of them Ito others. . Let me just say that it fortunate that I did not need a CT scan for a stroke based on how this case was handled. Admitted to my service when I was not on call. ER relied on a CT report on a cell phone...well there was plenty of other nonsense all compounded by his lack of cooperation to have testing done and the ER's admitting the patient with terrible information likely because he was abusive and they did not want him in the crowded ER yelling. A very experienced urologist agreed to do a cystoscopy and did it while the pieces of his story were being torn apart. When he had the cystoscopy and was told no stone no pain med is when he went on his worse of numerous tirades. He was no a kid, he was over 40. This may have been the most obvious drug seeking attempt I have seen in a long time and everyone just was bullied or turned a blind eye. This is not the usual level of care at this facility but it was one of the worst handled cases I have encountered.

I did not care what they said, but the tone and vitriol was disturbing and bothersome and as the question asks, disheartening. . The words themselves were rude and in the first case, would have been incomprehensible for me to imagine anyone saying. it is bothersome because as we go about our lives, we are faced with people who are so desperate and so thoughtless as to say these things. That one would not be disheartened by these statements suggests that perhaps we have become inured to the outrageousness of these kind of indignities. I have heard worse and had worse directed at me, shrugging it off to an acceptance that this is just the world we live in is a sad commentary on how far we have fallen.

I had a patient who became very angry that he was caught in a number of lies in trying to extract opiates. When I went to see him in the hospital this morning revealing that the tests confirmed that he did not have a kidney stone and so he would not be getting more pain medication and that he would be discharged from the hospital, he shouted at me: "hope all your kids get cancer and die in front of you, you fat sack of shit...I hope all your kids get cancer and die in your arms" Leaving there to go see another patient, an nondescript young man was running for the elevator and I pushed the button to hold the door for him. His reply was "Thanks Gramps I did not think you could move that fast."

On or off, I am one of those guys who has my eyes closed for much of the session. I get the picture in my mind and go from there. Lights on, eyes closed. I have to concentrate to keep them open and for some guys, it is worth the effort.

As I care for mostly indigent patients many of who are separated from their families, I have had to deal with conservators being appointed. In New Jersey it can take months to get someone appointed and Unicorn is correct in that in almost every case, there was a full court press to do everything, even when medical advice was to the contrary. In none of the cases with which I have been involved, did the patient improve as a result of extraordinary care. In one case, a distant family member who was found by the conservator, was living in another country and agreed to make arrangements for ongoing care there. The hospital rented a private medical evacuation plane to fly the patient and a staff of caretakers to the other country and care for him until he reached the other facility. As the patient was in a persistent vegetative state and he was only 24 years old, the cost, though exorbitant, was ultimately less expensive than caring for the patient for what might have been decades.

I for one am glad this thread was resurrected as I had an earlier post. That post from nearly 4 years ago speaks to me now as in the last 8 months I have had two dogs pass and now have two which are pretty ill. That post of mine reminded me that there is healing after the pain of loss. There are no coincidences, the individual pieces fit together and only once they are all together is the big picture seen.

I was having the NYT weekends (FSS) delivered to my home and then with the pandemic, the service became erratic, and I stopped my subscription. So my question is, Are there on the forum any avid newspaper readers who actually get a newspaper? For those newspaper readers out there, at what age did you start reading the newspaper? Just as an aside, at about 17 I taught my dog to fetch the newspaper from the lawn, where the delivery person always seemed to toss it. kismet was a great dog and she seemed anxious to do it.
