As Gilead Sciences prepares to launch twice-yearly lenacapavir pre-exposure prophylaxis (PrEP) later this year, the company is also developing once-yearly formulations...

Lencapavir currently shows 100% efficacy as a twice annual shot. Gilead, the company behind it, is working on an annual version of Lencapavir. I'm a condom nazi but the convenience of an annual PreP shot would make me reconsider my position.

Not knocking PrEP. To be clear, though, efficacy in the research cohort of cis-gender MSM and gender diverse persons was 96%, with lower bound of confidence interval 82%, relative to background HIV incidence. No randomized control group as non-PrEP study arm assignment deemed unethical. The few infected had capsid resistance mutations; lenacapavir is a capsid inhibitor. Low but not zero chance of breakthrough infection. The 100% metric pertains to a study of cis-gender women.

As much as I’d love the convenience of getting a single shot once a year instead of taking a daily pill, I do wonder how that might impact the spread of other STDs. Realistically, how many people would continue getting tested regularly if it’s no longer tied to a routine prescription refill? Let’s be honest, if regular testing weren’t a requirement for PrEP, a lot of people probably wouldn’t feel the need to do it as often.

I'd like to think that the day it becomes a reality isn't too far into the future, but I want to be alive to see the time that PreP or something similar can be taken while donating blood can be allowed instead of only being for opposite sex or non-sex individuals.

Almost all participants in the Lenacapavir study acquired an STI at least once throughout their enrolment. Extremely high incidence buried in the supplementary data and not mentioned in the published NEJM central text. Your point is well taken because for oral PrEP routine HIV testing is usually packaged with CT, NG, and syphilis screening. Incidence rate about 80 per 100 person-years means that an STI would be expected to occur almost annually per individual particularly for unprotected receptive activity. Gonorrhea (40 per 100 person-yrs) alarmingly about twice per 5 years.
