I had a pretty unexpected experience a couple months ago when, after I asked for his rate, an escort volunteered that the going rate in London was £200 for an incall, £250 for an outcall plus Uber, and that anyone lower than that wasn’t worth it and anyone over was trying to take advantage of me.

Totally varies. I get guys some booking ahead but most leave it to a couple of days before or even on the day. At the moment I’ve 6 bookings for the remainder of this month and 1 in September. Often I get asked if I’m free same day and usually I can’t do it because I’m busy.

Best to give a couple of options…could you do X or Y? Example: yesterday a client messaged asking about a morning session next Sunday or Monday. I could do one of those so it was booked.

I think its down to personal preference, both for you and the provider. My preference is to book in advance for London, but that's because I'm also only visiting and so like to line up my itinerary in advance. I'm also not the most spontaneous of people if I'm being honest so planning is just my thing. As an example, I'm heading there in a week's time and decided I could afford 2 meets. I reached out to two providers last week and have arranged time with them both. Hopefully, job done. But if either drops out, I've still got time to reach out to someone else on my wishlist. If you're using Rentmen, then the 'available now' might be useful if you like a last minute meet, but its obvious limited to those with immediate availability. Wouldn't work for me but might for you.

As @Jamie21 said, if you're booking in advance, be flexible and give a few options of dates/times and you should be able to arrange what you're looking for. I always offer to reconfirm the appointment the day before so the provider knows I'm still around, as well as confirming on the day when I'm on my way to them - offering both seems to go down well (no pun intended!)
