Then I guess the question to put out there is if you only tip for above and beyond the expected service, what do you expect the basic service to be? 
Do you think that everyone who advertises on Masseurfinder should at bare minimum offer a free happy ending?

How do you determine what is/should be included? 
If it is matter of your own finances, then surely there should be less expensive masseurs who you could choose and then tip accordingly?

I'm completely with you on the aspect of them giving a good massage.  There are few guys that are good a the massage part.  So if they've given me a solid massage with sensual or erotic aspects, those guys should be tipped. I use a traveling massage guy that keeps his rate at 140.00.  This guy travels to my city (and others).  He has gas, hotel and food expenses.  He's charging less than what the average to below average massage men are charging that live in my area and he delivers one of the best massages ever.  So yes, when someone is excellent and reasonably priced, you should tip them.  Even small amounts show an appreciation. 

He’s good to repeat clients; always gives an honest full hour, and has actual massage skills.  Friendly, good conversationalist.  Great guy.

I think the use of the word tip is confusing the point for some people.  A tip is "to insure proper service."  If you would like to ensure or see "service" increase on a future visit then a tip would be a good indicator to the provider.  Did they squeeze you in or take you as a last minute request... a tip is appropriate.
If you don't think most providers keep a record or payment,, services rendered and gifted amount, I believe you would be wrong.  Now... does it need to be 20%... not necessarily, but a little cash is much more memorable than a verbal gratuity and a hug.  Yes. the provider is keeping much more of the fee for the service visit (so it is not like you are paying someone that is working on tipped hourly wages) but I think a base $15 to $20 tip is expected.
 

Just tip for great service. If you can afford a $200 massage and expect some guy to throw in a hand job you can give him another $40 if it’s a good experience. 

Who said anything about bad service? No one. If you get bad service, don't tip - don't go back. What I said is that he was willing to do more for those who tip consistently. It only makes sense, if you have a choice between two clients wanting the same appointment time, I think everyone would choose the one who tips regularly. If a client asks for a prostate massage but never tips, why bother? But if they tip regularly the odds will go way up that the masseur says "Sure." Do you need to tip masseurs who run their own business? I guess not, but I appreciate good service and will gladly pay a few extra bucks to ensure it. I think it is reasonable to expect a good massage when you don't tip, but I also think it is reasonable to expect better service when I do tip regularly. 

Independent masseurs should ask the price they wish to be paid for the services rendered. If the services exceed the advertised labor, a “tip” may very well be warranted. If not, the advertised fee is sufficient. This approach has always worked very well for me. 
 
Let’s not play games. 

I believe that if you want continued great service (or increasingly better) service, tipping is a wise choice. I was chatting about tips with a local masseur who was honest that while everyone gets a great base massage, those who tip regularly get better service with scheduling preference and "extras" on the table. Just keep that in mind. 
Also with masseurs who work for themselves, know that they are probably paying at least 5% if you us a credit card or other electronic payments. 
I would think that it is important to tip when you are late, early, shower at their place, rescheduled your appointment or have any special requests like "keep the room hot" or "wear x clothing" or "I need to remain anonymous" or "no oils or scents" or music requests etc. I remember reading a bad review years ago from a client who apparently showed up 30 mins late (due to traffic and parking) showered at the masseur's home before the massage and was offended that he only got 75 mins on the table instead of 90 mins (even though he had now taken up two hours of the masseur's time). All I could think when I read that was it was the masseur who should be writing the bad review about the client and you know the guy would not have tipped either.

I'm 100% sure he doesn't offer erotic massages based on my experiences with him, but YMMV. He's good therapeutically though but normally doesn't offer sensual massages to strangers. He's a sweet guy.

If it is sensual massage and the fee is 180-200 I give tips ranging from 20 to 40.  If there is any fully interactive play time involved I pay a total of 300 because it was an escort session really.  I don't expect an escort experience at a massage rate.  I never have a problem getting multiple repeats and nobody has complained about my tips.
The above is for 60 minutes, incalls in NYC.
