As someone who LIVES here and travels frequently, you NEVER use a CAR to get to and from JFK unless its late at night, or you have tons of luggage. 
ALWAYS takes the Airtrain to Jamaica TRAIN station (NOT the subway) and arrive at Penn Station or Grand Central in midtown Manhattan. The cost is about 1/8 of a car service and avoids the risk of getting stuck in traffic (which is pretty much guaranteed).

How long is your layover?  Depending on your budget, one option is to rent a cheap motel room near the airport and get one of the Manhattan masseurs to do an outcall.
