    In all honesty seems like a great time to be patient and practice some restraint if you're a highly active man, keeping your encounters to a limited pod you feel comfortable and safe with until more vaccine gets rolled out and distributed. Seems like a small price to pay for some piece of mind

    Houston received an initial shipment of 5000 doses. A tiny amount compared to what other cities are receiving. 
    It was just announced today that an additional 16,000 doses are on the way

    Not a bad plan, given we have a limited supply at the moment, but it
    would be nice if we had a nation wide standard, instead of leaving it
    up to each state / locale to decide. The current system is all over the
    place and is leading to lots of confusion and frustration. Two things that
    you definitely want to minimize during an outbreak. 
    Even within New York, NYC is following one playbook and other areas of 
    the state are following different ones.

    Current HIV PrEP uptake is highly correlated with both risk and incidence of Monkeypox infection. As such, it is a clear identifiable dichotomous variable to integrate into the eligibility hierarchy, not unlike age wrt to CoV vaccination. Similarly, age wrt CoV was correlated with cluster transmission owing to settings housing the aged. This is analogous to PrEP recipients’ trends of congregating with like and stimulating forward transmission. The HIV PrEP subgroup priority is similar to PEP ring vaccination, but it is more anticipatory than established exposure to a confirmed case. The CDC is now categorizing this as Monkeypox PEP++, not PrEP. The latter category is reduced in scope and specific to occupational exposure such as lab work directly with orthopox viruses. There is nothing odd about PEP++ if supply is limited and mitigating incidence is the goal …

    We now know that seminal fluid (i.e. semen) is a vector of transmission for monkeypox. This is another argument for safe sex regardless of whether or not you're on PreP.

    A STI clinic? In your local DoH? Or a neighbor city DOH?
    A couple, friends of mine, went  for a routine STI screening, to a  NYC STI clinic /Sexual Health Clinic; both of them were offered and got the vaccine.

    LA County Public Health has several POD (Point of Dispensing) clinics where you can walk in without an appointment. Only prerequisite is that you should be on PreP. 
    But they do request ID, so I assume they also look for residency?

    Reading the LA Blade article it is clear that MPX will spread like wildfire in LA. Reminds me of how HIV spread in the gay community in the early 1980's, to which I was a witness in San Francisco and elsewhere.

    Wow, impressive article, I applaud the Blade and the author. 
    It was balanced, restrained, non-judgmental and gave good realistic advice. 
    My favorite line though….

    I was lucky enough to get the vaccine on Thursday July 21 at the Gay Lesbian Center in Hollywood where i happened to be for my regular STD testing. Surprisingly they were able to get me in and within 25 minutes I was vaxxed and out of there with a date for my second Vax in a month.

After my testing, I was in and out of that Vax clinic in 25 minutes. Organized, easy and a date in August for my second.

    Yesterday, here in LA, I had my first dose, which they offered during routine STD check.  No observable adverse effects at all, and I am very attuned to side effects (I find them interesting).  We scheduled the second dose four weeks from yesterday.

    In short, kind of, yes. The monkeypox vaccine give what can be generally called sterilizing immunity.

    To be extremely, extremely simplistic, a lot depends upon the virus, its site of infection/entry into the host, and the type of immune response elicited by the vaccine. Viruses are incredibly diverse, very cool life forms.
    For SARS-CoV-2, our current vaccines, while reducing symptoms of severity, don't elicit the type of immune response at the right virus' points of entry in our body, to prevent infection. And SARS-CoV-2 replicates like crazy once in the host.
    For monkeypox (and smallpox, and the unrelated viruses, measles, HPV etc.) the vaccines elicit immune responses that prevent infection, or slow it down sufficiently to halt it and to stop it from becoming symptomatic.
