Day One on PrEP

I had my first night on PrEP with no dreams I can remember. There was something weird this morning though. My cat's litter box was messed in a way that she never did it before and I do not think she could have done it. I am wondering if I sleep walked last night.

I think it takes a week for the drug to get to therapeutic levels, so I wouldn't expect any changes on one dose. My dreams have been vivid, but not scary. Abnormal just means unusual for that person.

You might want to install some cameras in your apartment. replay the footage the next morning and see what you have done. friend of mine sleep walks. scary stuff.

But then I have a question: if an escort says he's on PrEP and HIV-, but he uses a condom and the condom breaks, would a hospital or medical center be willing to offer both people PEP? Or would a hospital refuse saying that because one is one PrEP and both partners say they're negative, there's no point since HIV wouldn't be transmitted anyway?

 

I'm curious because since if an escort is on PrEP, they can't be HIV+ to begin with, but if you're on PrEP and the condom breaks, could you still take PEP if you or your partner is worried? Or does your doctor say there's no need for alarm?

 

I guess just because I see escorts says they're on PrEP and HIV-, but they're open for condoms, but if the condom breaks, what would your doctor say about that situation? PEP or no PEP? Would a hospital have problems giving PEP to people on PrEP?

I can't speak for a hospital, but from my standpoint there would be no reason to request PEP if I am using Truvada as PrEP. "PEP" stands for Post-Exposure Profilaxis. "PrEP" stands for Pre-Exposure Profilaxis. Why request the "after" when I would have taken the "before?"

Why would the hospital offer PEP to the person already on PrEP? They would just be giving him a double-dose of the same agent. The person not on PrEP would certainly be a candidate for PEP because he wouldn't have any way of knowing if the guy on PrEP was really on PrEP, or that his compliance was sufficient to keep him from gettting infected with HIV, etc. So many variables that are not within his control.

In this scenario, the person not on PrEP is certainly a candidate for PEP, no matter what the other person says.

My doctor if I remember correctly (actually it was a nurse practitioner) said I needed to wait two weeks before the medication would take effect. I still have a tendency to 'suit up' with strangers.

 

I currently have a FB (and I'm not talking Facebook). I'm fairly monogamous with him as regarding the topping situation right now-and I don't bottom. I do have a totally separate S (uck) B (uddy).

 

 

(I've been debating whether to start a thread to talk about the FB-but that's beside the point of this thread).

 

 

As for other side effects-I've had occasional vivid dreams all my life. I might have had a few more when I first started Truvada-but who knows. It might just have been my routine dreams occurring at the same time as my starting Truvada. I also had a vague nausea/dizzy sensation on first starting for a month or two. I am prone to vertigo so that might have been part of it.

 

There was one episode in the first month or two of being on Truvada. I was going out of town the next day. I had a lot to do, so I decided to stay up all night to get the washing done, do a bit of cleaning, etc. I don't think I had eaten a lot that day. Sometime around 4 or 5 in the morning, I developed one of my rare vertigo episodes. I grew light-headed, burst out in sweat, my blood pressure (if true to form from other episodes) drops, my temperature goes way below normal, and the room starts spinning.

 

(Years ago I presented to an emergency department with these same symptoms. I looked like death warmed over. They were afraid I might have a brain tumor-I had a CT and an MRI. )

 

I knew I had to get to sleep. I was able to change my plane flight to one that left later in the afternoon without a penalty. I went to sleep for 5 or 6 hours. I was pretty much ok once I woke up.

 

I had a feeling the episode was probably due to a combination of Truvada, lack of sleep, and possibly hypoglycemia/dehydration as I hadn't been eating or drinking.

providers seem to consider osteoporosis a very minor risk. Kaiser doesn't test for it, and I hsvent

Urinary n-telopeptide shows bone mineral loss at a very early stage, long before it is visible in any sort of imaging.

A good strategy. Truvada is effective in one week--for receptive anal sex. (bottoming) and 20 days for other sexual activities including vaginal and insertive anal sex (topping). Some researchers consider these numbers conservative and believe effectiveness is achieved earlier than these numbers.

Thanks. I went ready to do that about PrEP but when I explained my situation to her, she said "You need to be on this thing called PrEP". I was surprised then, so it wouldn't surprise me now if she knows about this.

Did she do the correct testing? Did she do an HIV antibody test and an HIV viral load?

Just the antibody test, it had been years since intimate contact with anyone, my first sessions that had already happened at that time were just body contact and j/o, no penetration of any kind, I was holding off on intimate contact until I got screened and could begin the regimen. One of the reasons for that on my part is I had a friend who died in the 80s who swore he had only ever done oral. Although my understanding is the general risk from oral is lower, every time I would blow a guy, I would remember him and have days of anxiety. Now the important thing is to keep up with regular screening.

 

But I should say I never saw a mention of a viral load tests in my own research, do you have a link? It makes sense and perhaps it would be good for everyone to have a reference?

Maybe I'm misreading or my logic chip is failing, but why would it take 20 days to protect a top and only 10 days to protect a bottom? Receiving without a condom (i.e. the top is bare) is one of the most risky things you can do without actually injecting yourself with the blood of someone who is HIV positive. Topping bare is much less risky.

I'm not sure the relationship shows me-or him in a good light (it's nothing illegal-we are both of age). And I started thinking-what if unbeknownst to me, he is also a member here. It's a small risk. But I didn't want to have to make up a lot of detail to shield the relationship.

 

 

 

Did she do the correct testing? Did she do an HIV antibody test and an HIV viral load?

 

I believe I had a '4th' generation combined antibody to the virus and antigen (=piece of the virus) test. But I'm not sure how quantitative the antigen part was.
