It means "biohazard." Not saying he is, but many HIV positive guys have it as a symbol of being poz. Not sure it's a badge of honor to put on full display.

Exactly... even though one should consider every partner to be a poz and take the precautions that one deems appropriate, there is something about knowing that a person is or might be that can be a bit off-putting. I say this also from the perspective of being a retired health care professional and seeing how various staff members reacted... and emphatically not in the sense of being prejudiced or disrespectful. However, it can put one on edge such that one might tend to be overly careful and in the case of an escort situation put a damper on the enjoyment factor. At least that is the case for some.

on a profile he has on a hookup site he states "HIV positive and staying that way" or something to that effect. He is openly poz and the tattoo reflects that.

We're getting close to the privacy line here guys. That tattoo is a +1 in my book, as it opens a door to communication. I'm very sure that if you ask him about it he'll be forthcoming.
