If you’re willing to go to Brooklyn I strongly recommend MercuryRising.  He may be willing to come into Manhattan if you pay.  He mixes massage and some Zen like elements and once he knows you want extras he delivers.  I have seen him many times and he is very gifted and has amazing passion. Well, tipping is optional but expected - except when being serviced by the proprietor of a business. If I’m at a spa, I’m tipping, otherwise I expect the quoted price to be the price. However, a good massage can be hard to come by and I think generosity generally helps when the service is worth it

Correct. The rates quoted are for strictly therapeutic. When you posted his rates you hadn't specified that they were limited to therapeutic only. That makes a big difference. He's also $100 to come to me not that far away in Manhattan. That's a lot.
I was turned off (similar to Bozo) with the rest and decided not to book. Maybe others aren't and wouldn't be. Each can decide on his own.

As for how much to give in gratuity, I don’t charge extra for options 2 and 3. I give each a chance and leave it to each to decide how much to give, how generous or not to be, and whether to provide what is fair.
 
For guidance on prevailing rates for sessions that include options 2 and 3 in the specific area of New York City, I highly recommend asking the competition in Manhattan (the few who have consistently delivered high-quality sessions and earned a good reputation) what they charge for options 2 and 3.
