I haven't had an 'accident' for years, but there is a point at which you have to accept that when playing with fire, you will occasionally get burned.

Well as mentioned I normally have a very high fiber diet and as such am quite the "regular" guy. Last week for some strange reason I bought a carton of prunes, which I had not had in quite a while. Well, in retrospect I overly indulged. I didn't have an accident, but I could have had different types of accidents as things were flowing a bit more swiftly and unpredictably than usual. However, let's just say that I could have had a colonoscopy as I was way beyond being prepped and cleaned out. Again a total waste of GI functions.

Spitting. IMO spitting is hot about 5% of the time in porn, and 0% of the time in real life sexual encounters.

 

Rimming scenes are often hot in porn, with enthusiastic rimmers eating perfect asses. I wouldn't consider rimming most of the men I've met.

As @escortrod says above, accidents happen, even when your GI is functioning perfectly. If you're into fisting or very long fuck sessions or top's who are hung very long (9" and above) then douching is well advised. But, otherwise, douching shouldn't be part of a regular "prep session" because it does dry out these very delicate tissues over time and could lead to damage or other issues. And that assumes you are bottoming at least once a week. If you only bottom a couple times a month, douching should be safe so long as you're just using warm water.
