Not that I have any inside knowledge, but yes, this. I have to think adhering to Dan Savage's three Gs -- good, giving, and game -- helps, too. I assume the point of disclosure, requested or otherwise, is to avoid surprises and, possibly, as an informal way to verify that the person who shows up is the one who set up the appointment.
