I am careful what I do with an escort and always assume he has not been careful with other people 100% of the time regardless of what he states in his ad. I interpret "anything goes" to mean I don't necessarily have to go there, and "safe only" to mean that he is being careful now, which says nothing about what he might have done in the past. I always assume the worst and play accordingly.
