Horrible medical mishap is a bit overblown. Two hundred forty people were informed that they had colonoscopy with a scope that had not been properly sterilized. It is not clear how many, if any, were exposed to HIV Hep B and Hep C as they do not mention that any of the viruses were found in the water channel in the scope nor that there was any other proof of actual viral exposure The hospital is seeking to test the widest possible exposure risk and checking those patients, probably to limit liability.

Here in NJ we had a situation with a doctor, an oncologist no less, reusing needles to draw blood. 3200 people were notified to be tested. No cases of HIV Hep B or Hep C were found. It was a medical mishap, but it does not seem to me it qualifies as horrible until someone has a significant consequence from the event. Of course in this case, the doctor knowingly practiced medicine in a way that put his patient's at higher risk and he had his license suspended.

I remember exactly the same thing occurred in Belgium several years ago. A hospital that wasn't using proper sterilization for colonoscopy equipment.

And apparently something similar occurred at Miami VA hospital in 2008-2009

The precaution is to test everyone who had a colonoscopy. But don't panic. Taking things into perspective it's extremely low risk for HIV transmission.
