I have not had anyone turn down a tip, but I find the more often I hire, the less often I tip, as I agree that the escort sets his fee. Secondly, I usually do something above and beyond for the escort whether it is a meal or a small gift. So if I would have tipped, I do a bit more for the meal or on a future date, might make the gift a bit nicer.
