When I finally received the intake form I was within the 24 hour cancellation period. He's not a doctor. I have not actually met this guy, so I can't speak to his actual skills, but in my opinion I would avoid. From what I have been told its a great massage and if he's into you, you will be having A LOT of extra fun.
