This is probably a fundamental question. Let's say you plan to be with someone over a weekend and you know that each weekend day will contain activity that you would like to be protected by 2-1-1 PrEP. So from reading the thread, if you consider just one of the days you would take 2 pills optimally 24 hours before and then 1 pill each of the days after. But how about that middle day. I have a feeling you would not take 2 pills 24 hours before that. Does the first two pill dose cover that (followed by one pill per day after that)? Or is there some other set up you would recommend. Thanks.

2-1-1 PrEP is only for anal sex. Take 2 pills 24hrs before your first sex session. Continue taking 1 pill, at the same time, every day as long as you're engaging in anal sex. Once you've had your last round of anal sex then continue taking a daily pill at the same time for 2 more days.

hi, so you take a pill every 24 hrs after the 1st double dose. below is a hypothetical pill schedule for a friday start and sunday end (meds are effective until 1pm monday):

 

2-1-1 weekend schedule:

friday 1pm: take 2 truvada as directed

saturday 1pm: take 1 truvada as directed

sunday 1pm: take 1 truvada as directed

 

the double dose from friday flooded my body with an enzyme blocker to protect me against exposure to hiv. the medication is completely active & 96% effective with no decrease in protection if i take the the 2nd dose (1 pill) as recommend on saturday at 1pm. on sunday at 1pm i take my 3rd dose without any dip in the enzyme blocker....enzyme blocker has no dip in effectiveness until 1pm monday.

 

99% protection against hiv after 7 days of truvada (7 doses) when taken as directed

96% protection against hiv after 3 days of truvada (4 doses) when taken as directed

76% protection against hiv after 1 day of truvada (double dose) when taken as directed

 

consider also if your partner is on PrEP or positive & undetectable and in both cases taking the meds as directed...you start getting into virtually impossible situations for infection

 

obviously you can adjust your pill schedule to start at anytime you want. do what is right for you and your schedule.

 

also, i gave an example of how i might use 2-1-1 on a weekend with someone i am dating on an earlier post. i used dating as an example b/c that means we talk openly & often about our sexual health, the methods of protection we use & any risks or concerns we may have for each other. i would start the pill at a totally different time (and day) if i'm booked for a 3 hour (min) group session at a client's hotel on a friday night at 8pm.

 

doctors recommend 4 weeks on truvada from the day of any significant hiv exposure - i've done this twice via PeP

That is as much an issue if the patient's ability to speak as am issue with the doctor. BTW you don't need to tell them explicitly you're seeing providers. Just that you are not in a steady relationship and some of your partners themselves have a lot of partners. That's enough that a competent doctor should know you are in the higher risk category.

I talk to my doctor about Everything. He even knows I use providers. After someone sees your anal warts, there aren’t many secrets. And yes, he is gay. If you are not comfortable talking about your inner most secrets with your doctor, no matter how great you think he/she is, you need a new doctor.

 

But if one only gets penetrated when the partner uses a condom, chances of getting anal warts is pretty small, right?

Warts are skin to skin transmitted. You'd be protected from getting internal warts, but skin around the dick is likely coming into contact with the skin of your ass, and you cpuld grt them on the outside. So get the shot if you haven't.

But it seems they only give HPV vaccines to people of certain age.

In late 2018, new research came out that led to the FDA updating the age limits for 9vHPV. By June 2019, the ACIP recommended updating the age range and the CDC agreed. For everyone getting vaccinated after 15, the recommendation is a 3-shot series. If you start the series before turning 45, it should be fine for your to complete the series, but your insurance may not pay for them, so plan accordingly.
