I think a follow up email might be a good idea. Tell him how much you enjoyed the massage, maybe that you always enjoyed massages from him and found them relaxing, but you felt even more relaxed outside the clinical setting and that you look forward to seeing him again soon.

I know many will disagree, but I think the ability to deep throat is more biological than an acquired behavior. I am not saying that one cannot acquire the ability, but I think it's easier for some than others. I was a late bloomer and didn't start sucking until I was over 40, but I was able to deep throat pretty quickly.
