There is no logical reason the escort needs the information that he asks for in his questionnaire. Any client that answers the personal questions asked makes the client extremely vulnerable to any whim of the escort. Privacy and/or confidentiatity of the client disintegrates upon submission of a completed questionnaire.

I have seen only one other escort use a similar form. That escort is highly regarded and he posts to this form. I know that sometime last year he was made aware that clients do not find that interrogation type questionnaire from an escort appropriate. To the escort's credit, he removed the requirement from his ad. I suspect he lost a lot of business because of the questionnaire just as I suspect you also will lose business until you remove that questionnaire requirement from your ad.
