I read it as, the pics are current, but he used to carry 10 lbs of more muscle. Apparently the escort mentioned that he used to be 10 lbs heavier, and the reviewer was mentioning that he wished he would put the 10 lbs back on, since the client mentions in one sentence that he felt the escort was a little lean.

 

10 lbs of solid muscle is really quite a bit, he would look much different if he did add that much muscle. IMO
