I am a little confused here.

 

Are you interested in having access to cutting edge research or are you wanting to hear information heard through the grapevine?

 

If what you are really interested in is factual information about HIV I would kindly suggest that you visit pertinent websites and scientific journals and learn directly from the source. You can compare findings having in mind research that has been done in subjects that live in similar conditions to you and try to avoid research that has been done studying people in entirely different conditions. (It would not be useful at all to read studies conducted in Africa amongst prostitutes that never use condoms and the way circumcision has reduced the risks of transmission. If you are a gay man living in a Northamerican metropolitan area engaging in specific sexual practices, it would be more helpful to read studies and seek counsel from specialists who are familiar with such cases.)

 

Escorts as well as adults should be very well informed about the safest prevention techniques, should know about pre and post exposure prophylaxis and accurate risk assessment, and comparing notes in this respect could be helpful; however, when it comes to actual virology you would be better off getting your information from reliable and professional non-biased sources.

I certainly think this is a topic that is worrisome to me and has been for quite some time. I would atually be surprised if there weren't going to be some variants of HIV that will be develop drug resistance. How many other viruses/bacteria develop resistance to drugs used to treat them? Most of them, I wager. That's one of the concerns I have with so many people, younger men particular, thinking that if they get HIV they'll just take the drug cocktails -- no problem. The apparent rise of more and more guys willing/wanting to bareback is troublesome because they view HIV so cavalierly thinking the drugs will take care of everything. What happens if they don't?

 

I haven't heard of any incidence of drug-resistant HIV strains, but I wouldn't be surprised if/when we do.

In another thread, one commentator bravely theorized that escorts and their clients by nature of their commerce "exchange diseases." This was a very cogent statement, and I wish others had followed up on this.

The "Journal of Antimicrobal Chemotherapy" is published by Oxford University Press, a world-class publisher, on behalf of the British Society for Antimicrobal Chemotherapy," one of the world's leading scientific societies. A further description of the source journal is below. If this is considered a biased source, more information is needed as to why. What examples, if or in additional to this, would be regarded as reputable sources, beyond the public websites of NIH?

 

In regard to the relevance of population samples from overseas, I am personally worried about the spread of drug-resistant HIV from overseas populations to the USA via sex tourism. This is only a personal worry, and I do not mean to imply that this study deals with such a trend. It would seem to me, however, given that gay men travel abroad extensively, that any infectious disease eventually travel internationally.

 

I'm not certain, but I believe certain cruise ships which zero in on the gay market, might actively involve entertainers "in the profession." What steps are these companies taking to educate both their employees and customers about public health? I've not taking any of these trips, and would welcome feedback and commentary from those who have.

 

Certainly many escorts must involve themselves in lucrative, extended overseas travels with clients.

 

Munroe and Ross are correct that I like to post provocative information.

In addition to this, I hope this material that will also raise issues that deal with everyone's concern: their personal health, their quality of life their ability to work and earn money, their responsibilities towards their friends and colleagues and clients, their ability to engage in sex with peace of mind.

Any additional replies to this thread would be welcome. I would like to hear, with all respect and seriousness, about what websites and scientific journals are considered reputable as opposed to those which are not, and the criteria for their selection. Everyone's sexual health and pleasure is everyone's business here.

 

I hope readers will be OK if I provide another reference to drug-resistant HIV.

In the September 2011 issue of "Current Opinion in HIV & AIDS" (March 2011, Volume 6, Issue 2, pp. 131-140), appears the article "What do Mathematical Models tell us about the emergence and spread of drug-resistant HIV?" The publisher is Lippincott/Williams & Wilkins, who are one of the world's leading medical publishers.

 

You don't have to be a virologist or mathematician to think that the situation is fine and dandy in regard to drug-resistant HIV. It is not.

 

Time and time again, the wisest, most educated escorts and the finest, most brilliant clients on this website have decried barebacking. There are other forms of unprotected sex. What do mathematical models tell us about the emergence and spread of drug-resistant HIV?

Purpose of review: To discuss recent HIV epidemic models examining the transmission of antiretroviral (ARV) drug resistance.

 

Recent findings: A relatively small number of recent transmission models have investigated ARV resistance in the context of therapeutic, combined ART (cART); ARV-vaginal microbicides (ARV-VMB); and oral pre-exposure prophylaxis (PrEP). Models of cART use have highlighted potential concerns about future resistance transmission, particularly in resource-constrained settings, and have emphasized the benefits of viral load monitoring in limiting resistance spread. PrEP models have concluded that inadvertent use by HIV-infected individuals could increase resistance prevalence, and that risk compensation by PrEP users could limit their beneficial effects on HIV transmission. ARV-VMB models have demonstrated that whereas resistance can reduce prophylactic effectiveness in preventing HIV acquisition of female ARV-VMB users, it may concomitantly benefit users' male partners if the resistant strains that female users acquire are less transmissible than wild-type strains. The models have examined the balance between these two factors at the population level.

 

Summary: Recent HIV transmission models have adopted a wide assortment of structures and assumptions to explore drug resistance in the context of different ARV interventions in various settings. There is a need for future work emphasizing the simultaneous effects of multiple ARV interventions, as well as the public health impact of resistance, not just its prevalence.

I'm a bit unclear as to the real intent of your question. Drug-resistance in HIV is a well-known issue, having been reported for years (in the US as well as elsewhere). Google Scholar produces numerous articles dating back years. If your intent is to raise awareness, that's admirable, but I suspect everyone here knows this already.

 

As for seeking anecdotal reports...I wouldn't have thought it's the type of thing a patient would broadcast.

 

So I'm guessing it wasn't a real question but intended to provoke discussion?
