The guy has well developed muscular breasts that look as hard as a rock to me. Some guys happen to have some extra tissue in the breast area and if not worked out to harden the tissue into muscle, could truly look like hanging male breasts. It happens especially when a guy is chubby as a kid and the extra breast tissue remains after puberty although the flab disappears in other areas of the body.
