Your cell phone should be off when you are with a client. You can always listen to and answer messages when you leave the client. If it's an in call you should wait until the client leaves. IN my opinion the phone should be off during an appointment. It just works better for everyone.

You might think that you are perhaps missing the opportunity to make a new client if you don't answer the phone, but if you do, you might be loosing the chance to turn this current client into repeat business. Besides answering the phone is just rude.
