1.) it is a sole-propietership located in San Francisco (the owner being the top in the vid).

 

2.) The top ("Roger") is highly skilled and the bottoms know this and trust him not to do permanent damage.

 

3.) The bottoms are really into it - they are not general porn actors willing to do anything for sufficient compensation.

 

So, even if I knew of escorts who happened to enjoy receiving ball work in their personal life (which, unfortunately I don't), it doesn't necessarily translate to them trusting a stranger to hurt them without damaging them.
