The hospital about which I am speaking is one of the most prominent cancer research hospitals in the world. It is a leader in da Vinci Surgical System (robotic) and most likely the most respected hospital in the U.S. in the area of blood borne cancers. The doctors, the nurse practitioners, the RN's, and the technicians all appear to be outstanding. Where the system tends to fall apart is with the support secretarial staff. I'm convinced many of these men and women "failed my photography class".

Frankly I have run into the same problem with business. The larger the business the greater the problem. I assume the middle and upper management know what they are doing but again many of the first contact secretarial staff appears to be made up of men and women who "failed my photography class".

Dont assume advanced medical technology means 21st century coordinated tech.

May seem counterintuitive... but as each medical subspecialty developed highly specialized tech equipment, that equipment doesnt alway play well with others. And superstar doctors, often older, HATE administrative tech. 70% of the older doctors I know will tell you they think electronic records are a good thing, but they hated the training, hated modifying their practice, and it made them less productive.

The scheduler in imaging may be scheduling with a different system than the scheduler in radiation therapy.

Healthcare delivery is one of the most convoluted, illogical business models imaginable. The bigger the center, the more complicated. Add in University affiliation, faculty and oversight.... it would be funny if it wasnt tragic.

We live in a would of adapt or die.

When I started teaching high school in the 1960's we used chalk boards and ditto machines. When I retired in 2002 we used computers and the internet. When I started teaching roll books were kept in hand written books not on computer programs. Many of the older teachers did everything they could to stall the changes but in the end they either adapted or retired.

A lot of older doctors I know hate all the technological record keeping and the time it took for them to learn the system. They still wanted to have hand written records that were kept in their own offices. Those days are gone. Hand written records are a thing of the past computerized records are now kept in central repositories readily available to other doctors and heaven forbid patients. Adapt or retire.

Two years ago, at age 74, I finally bought a smart phone. I hated the idea of texting and keeping my calendar of a damn phone BUT I learned and now love the system. Adapt or die
