I wouldn't go with the tshirt if it were me, I'd just buy the underwear. Size wise, guess it depends how tight you want it to be on him! Haha!

An extra large t-shirt. What is his waist measurement? Probably large shorts/trunks. I think small gifts like that are always appreciated. I only had one escort return the gift to me and ask for the money. They were some rather expensive shirts that I had order especially for him.
