Giving the middle finger (or two middle fingers as in this case) means "fuck you" - and not in the "love-making" sense of the word "fuck." Although this gesture has become more commonplace in the last few years, it still behooves me how an escort (real or fake) is so deluded as to think that this gesture is not off-putting (if not borderline hostile) and then to use it in a photo meant to attract clients
