Without doing a drug screen, one cannot tell whether a psychotic person is schizophrenic (in this case it would be schizophrenia, paranoid type) or if the person is on drugs (commonly amphetamines, others possible), or if the drugs are making the underlying psychotic disorder worse. All patients admitted to our psycho ward get urine drug screens (although some drugs, especially mushrooms, do not get detected with standard urine drug screens). I would have to seriously question the judgment of someone who would invite an obviously disturbed individual to dinner, and would have to question the sheer sanity of getting into his car. If the escort's mental health issue is not just a temporary/drug issue, he may have dabbled into escorting and porn because his mental health disorder precluded holding down a regular job. Some schizophrenics will qualify for social security. Escorting can provide additional income which they may not report to the IRS or Social Security Administration, and therefore let them continue to collect said social security. I do see this in my practice, although it's usually women (I work in an area without a big gay population).
