I don't know if you have to have EVERY one of your criteria met, but if you have some flexibility I would whole-heartedly recommend Danny in Nashville. He:

 

(1) is preppy (21 yr old college senior) - he reminds me of the interns hired to work at my office--looks really hot but you assume he's straight.
