Hi there, if you are still looking I would recommend Drew in Tampa - https://rentmasseur.com/drewintampa.  He has a great setup in his home (table, private bathroom, etc.) and he checks nearly all of your boxes except for the hairy chest part.  Nice guy, very reasonable rates.  Good luck!
