I don't think it's quite just effeminate. In many cases, these ladyboys may have breasts. They definitely want to pass as a woman on the street - not just feminine. But they keep their cocks.

I know what you mean - my last trip to Thailand I noticed it specifically; very common, some places even have separate restrooms for them. Sorry to say I have not come across any at NYC spas.
