If you've read the ETIAS information that @mike careyprovided, you should have all the details. You apply for ETIAS authorization online. When you receive the approved ETIAS authorization, it's good for three years (or until your current passport expires). It's electronically connected to your passport number, but it doesn't appear in your passport.
